Athletic Insurance/Injury FAQ

1. Do I need YU insurance to play a sport at Yeshiva? Will my own insurance qualify me?

You need to have health insurance in order to play athletics at the University. You can be covered by your own insurance or that of a spouse or parent, or you can opt into the University’s coverage through Academic Health. If you opt for University insurance, it is critical that you also alert the Risk Management office (riskmgmt@yu.edu) so that additional coverage specific to athletic activities can be added to your plan. If you are insured by your own plan, you should read the policy to see whether athletic injuries are discussed. If they are excluded, you may not be covered.

2. Am I covered under the athletics insurance if I am playing intramurals? What if I am playing a pick-up game?

If you are playing intramurals or pickup games, you are not covered by the athletics insurance offered by Yeshiva.

3. What forms do I need to fill out in order to participate in sports?


If you are interested in playing a sport, please fill out the correct form on the Athletics Forms site. There is one form for returning athletes, and another for new athletes.  You must take the form to the health office on campus to be reviewed and stamped.  No forms will be taken without a stamp from the health services department that indicates you have had a physical within the past 6 months.  If you need to schedule a physical, please contact that athletic trainer for more information.

4. To whom should I submit the forms?

Once completed, bring in all forms to the athletic trainer.  The training room is behind the gym near the large classroom.

5. Am I covered for an injury if I am traveling with my team?

Yes, if you have YU health insurance, team travel is covered.

6. What forms do I need to fill out if I get injured during a team event?

The Intercollegiate Sports Accident Claim Form and Yeshiva University Report of Sick or Injured Student.


7. How long will it take to process the bills after I hand them in?


In order for bills to be processed, the incident report and the claim form must be filled out and signed by the athletics trainer. This paperwork must be submitted as soon as possible. In order to expedite the process, you should also take the claim form to the medical provider who can submit it directly. All bills must be received by the end of the academic year, as we are unable to process bills after June 30.


8. What is the process for seeing a doctor for a non-emergency athletic team injury?

Please complete the incident report and the claim form and make sure the athletics trainer signs each. This paperwork must be submitted as soon as possible. The athletic trainer will help you find the right doctor for your injury. In order to expedite the process, you should also take the claim form to the medical provider who can submit it directly. All bills must be received by the end of the academic year, as we are unable to process bills after June 30.

9. Why do I need my own insurance? Shouldn’t the school pay for everything?


Unfortunately, the University is unable to pay all medical expenses. Since participation in athletics is voluntary, it is incumbent upon the students who choose to play sports to make sure that they have appropriate insurance coverage.

10. Can I see my own doctor?

If you are covered by the University’s plan, you can see your own doctor as long as your doctor is in the network. If your doctor is not in the network the University will not be responsible for the charges associated with your visit. If you are unsure about whether a doctor is in the network, you can consult the health plan membership card or contact the Office of Risk Management at 212-960-5400 extension 5340.

11. What should I do with medical bills I receive in the mail?

If the bills are for injuries incurred while participating in Athletics, you should bring them to the Office of Risk Management along with the incident report and the claim form, which should be completed legibly and signed by the Athletics Trainer. Please note that all medical bills must show exactly what service was rendered. Therefore, if your bill says “Balance brought forward” or similar language, you should request a UB90 from the medical provider. This form cannot be requested by the University due to privacy laws, so it must be requested by the patient.

12. Am I responsible for co-pays?

You will need to pay for the co-pay at the time of service. However, if you are able to provide proper documentation to support your request, the co-pay will be reimbursed later.

13. Does the University insurance cover dental expenses?

The YU Athletic Accident Insurance will cover injuries to the mouth and natural teeth incurred while participating in an official practice or game. Dental care not directly related to athletic injury will be the responsibility of the student-athlete.



IMPORTANT, PLEASE READ

Reporting Procedures:
The student-athlete is responsible for reporting all injuries to the Athletic Trainer as soon as possible. The Athletic Staff will complete the necessary injury report form.

In the event of an emergency (due to an athletic injury in a formal, official game, practice, or travel) the athlete will be transported to the nearest, most appropriate hospital or medical facility. The student-athlete will at no time seek outside medical attention for an athletic injury without prior consultation from the Athletic Department, Risk Management, or the attending physician in the case of an emergency.

In the case of HMO/PPO policies, the student athlete may be required to follow specific referral procedures. The University, and the Athletic Department will not be responsible for any charges incurred if these procedures are not followed.
